The FDA announced it has approved a 3-year expiration date for ropinirole ophthalmic solution (Clevor; Vetoquinol), when stored in its original packaging. The approved extension will increase the product previous expiration date by 12 months to 36 months from the date of manufacture.

“Similar to morphine, [ropinirole ophthalmic solution] does have ability for reversal [with] just a dose of metoclopramide. And luckily, you get them to stop vomiting because oftentimes the next treatment is activated charcoal. Giving a patient who’s already vomiting activated charcoal can sometimes be difficult, so it’s important to be able to get them to stop vomiting as quickly as you get them to start,” Erik Zager, DVM, DACVECC explained on a previous episode of 

Ropinirole ophthalmic solution is indicated to induce vomiting in canine patients and is the first, and currently only, FDA-approved product for emesis induction in dogs. According to a release from Vetoquinol, the ocular administration of the project is unique and makes the induction process easier and injection free. This can result in a less stressful process for veterinary professionals, dogs, and patients.

Ropinirole ophthalmic solution comes in little individual packets and to use them, veterinary professionals can open them and use the provided little eye dropper. Each packet tells you have many drops go in and whether to split between 2 the eyes, patient depending. The vomiting will usually happen within 20-30 minutes. If vomiting does not start, veterinary professionals could administer the dose a second time. However, the drug works quick and efficacious to get them to vomit.

This new extension was granted to ropinirole ophthalmic solution based on the completion of an extensive product stability testing and there has been no change to the formulation. Any veterinary clinics currently in possession of Clevor lot number EM099 now has an additional 12 months of expiration.

“We’re past syrup of ipecac at this point. We got into apomorphine, [but] having an FDA-approved drug [now] is great. Sometimes it’s a little difficult to get [away from] what you’re used to, but veterinary medicine is about making sure you’re up to date, embracing new products, and advancing your practice every day,” Zager said in his interview.

FDA extends expiration date for solution to induce vomiting in dogs

Dechra Limited, a specialist in veterinary pharmaceuticals and related products business, has purchased Akston Biosciences Corporation’s long-acting veterinary insulin programs. The purchase gives Dechra—a global subsidiary of Dechra Pharmaceuticals Limited—ownership of the intellectual property and rights for the development, manufacturing, and worldwide commercialization of the canine and feline candidate therapies.

“The sale of our interests in these programs is the culmination of years of product development from the selection and optimization of lead candidates to successful proof-of-concept studies in diabetic dogs and cats, and finally to process development, GMP manufacturing, and scale-up to commercial production levels,” Todd Zion, PhD, president and CEO of Akston Biosciences, said in a news release.

In 2019, the 2 companies signed a licensing and supply agreement for the co-development and commercialization of a long-acting insulin for the treatment of diabetes in dogs that Akston invented. A similar agreement for a long-acting insulin for treating cats with diabetes was signed in 2021.

Developmental products included in the partnership include Akston’s most advanced animal health candidates, the once-a-week insulin therapies AKS-321d for canine diabetes and AKS-425c for feline diabetes.

 According to Akston, these product candidates have the potential to significantly improve the current standard of care, while enhancing the quality of life for pets with diabetes and their owners by replacing twice daily injection regimens with a once-a-week insulin therapy.

 These longer-acting therapies are not yet available on the veterinary market and a timeline for their commercial availability has not been disclosed.  

Patrick Meeus, DVM, PhD, DACVM, chief scientific officer of Dechra, said the company is enthusiastic about commercializing the newly acquired insulin therapies for treating diabetes in dogs and cats. “The acquisition of these breakthrough programs is a great example of Dechra’s commitment to improving animal health and welfare through innovation. We thank Akston for all their hard work and dedication in helping us bring these innovative products closer to reality,” Meeus said in a news release.

In exchange for its veterinary insulin programs, Akston is receiving an upfront cash payment, fees to support technology transfer, and near-term milestone payments based on the program's development progress. “We look forward to Dechra taking these programs through late-stage clinical trials and commercial introduction while Akston focuses on developing our strong pipeline of animal health product candidates in obesity, cancer, chronic pain, and atopic dermatitis,” Zion said in the release.

Development of both veterinary insulin therapies is based on Akston Biosciences' Ambifect Fc-fusion protein platform, which has enabled the company to develop therapeutic candidates that interact with parts of the immune system related to the target disease and minimize the effects off-target. Through this technology, therapeutic development time and manufacturing costs across its pipeline can be reduced, according to the company.

During the summit, hosted by the KC Animal Health Corridor organization, Zion presented Akston Biosciences’ work on developing these novel canine and feline diabetes that rely on monoclonal antibodies. He noted that the company had invested in its own manufacturing facility and was leveraging technology to find cost-effective, efficient methods to address various needs in the animal health space. “We really believe that the veterinary field health field is poised to experience its own revolution in this area,” Zion said at the summit.

Global veterinary pharmaceutical specialist acquires innovative insulin programs

The World Veterinary Association (WVA) is seeking experts on rabbits and poultry for the Essential Veterinary Medicines List (EVML).

 The EVML is the world’s first global list of essential veterinary medicines for food-producing animals, with nearly 300 medicines across 11 categories that include antimicrobials, vaccines, hormone therapies, and metabolic agents.

The EVML aims to enhance access to safe and effective medicines and vaccines for veterinarians globally. It will serve as not only a framework for countries to create their own customized lists, but as a tool for addressing the global challenge of antimicrobial resistance.

 It will also aid in the development of preparedness plans for pandemic prevention of zoonotic diseases like avian influenza and COVID-19, according to a news release.

“This list is the product of global veterinary expertise and collaboration and is a critical first step in ensuring all animals around the globe have access to medicines and vaccines to keep them healthy and minimize their pain and suffering,” Shereene Williams, senior manager of global animal health at Brooke—a global equine and donkey charity—said in an organizational release.

According to Brooke, it is estimated by the World Health Organization that approximately 2 billion people lack access to fundamental medicines, and it is believed that even more animals do not have access essential medicines.

 Moreover, a survey conducted by the creators of the EVML found that 80% of veterinarians said that challenges in accessing veterinary medicines limit their capacity to address animal health and welfare.

Developed by the WVA, Brooke, and various species specialists who aim to advance animal health, the EVML is comprised of “core medicines” and vaccines chosen by expert groups based on their relevance, effectiveness, and cost-efficiency.

 Phase 1 of the initiative addressed the essential medicinal needs of equids, large ruminants, pigs, goats, and sheep. Phase 2, which is currently ongoing and seeking specialists, concentrates on poultry and rabbits, and Phase 3 will expand to include aquaculture and bees. Although the lists are tailored to specific species, a central multispecies list was created to assist veterinarians in treating a range of animals.

With Phase 2 currently in expansion, the WVA is seeking specialists in rabbits and poultry to help in the list’s review process. According to the organization, the role includes reviewing and providing feedback on draft lists of essential medicines for these animals, with a time commitment of a few hours fulfilled electronically. Moreover, reviewers will be recognized in relevant EVML materials, according to the WVA.

“The EVML will help veterinarians and authorities in making better choices of medicines, biologics and vaccines supply, fitting to local needs,” veterinarian Olatunji Nasir, pharmaceutical stewardship working group chair of the WVA, said in a news release.

 “As veterinarians, we are gatekeepers of the next pandemic because of the profound roles we play in the control of zoonoses. This is a responsibility that we share with authorities and agencies in our various jurisdictions. Together, we stamp our feet in the one-health pathway.”

Given the EVML is an open, collaborative, and free resource, the WVA welcomes participation from the wider veterinary and scientific communities, according to the organization.

Global list of essential veterinary medicines for food-producing animals in need of rabbit and poultry specialists

According to the United States Department of Agriculture (USDA), rabies poses a serious public health concern. Approximately 90% of rabies cases reported in the US come from wildlife, per the CDC.

Moreover, rabies is almost always fatal after symptoms begin. Still, this viral disease is 100% preventable.

Working towards preventing the spread of wildlife rabies in the United States, the Animal Plant and Health Inspection Service (APHIS) has announced that it will continue field assessment of the oral rabies vaccine bait—ONRAB—in 8 states as part of its collaborative broader rabies control program, titled the National Rabies Management Program, which aims to eventually eliminate terrestrial rabies in the country.

This year, the assessment will focus on examining ONRAB’s distribution techniques and its effectiveness in raccoons, skunks, and other wildlife species. The ONRAB rabies vaccine, coated with a sweet attractant to act as a bait for wildlife animals, is a blister pack filled with the liquid vaccine. The vaccine entices animals with a sweet coating made from vegetable-based fats, wax, icing sugar, vegetable oil, artificial marshmallow flavor, and dark-green food-grade dye. When animals bite into the bait, the vaccine gets released into their mouth, enabling them to develop immunity from rabies. According to the APHIS, the vaccine has been successfully used in Canada to immunize raccoons, skunks, and foxes against rabies.

Starting in late July, the lead of the National Rabies Management Program—APHIS’ Wildlife Services (WS)—and its partners, will deploy approximately 3.3 million ONRAB oral rabies vaccine baits via airplanes, helicopters, and vehicles across regions in Maine, New Hampshire, New York, Ohio, Pennsylvania, Vermont, and West Virginia to test the vaccine's impact on targeted wildlife.

Throughout August, the distribution of this oral vaccine bait will cover parts of

Cattaraugus, Chautauqua, Clinton, Erie, Essex, Genesee, Jefferson, Lewis, Niagara, Oneida, Orleans, Oswego, St. Lawrence, and Wyoming counties in New York

Addison, Caledonia, Chittenden, Essex, Franklin, Grand Isle, Lamoille, Orange, Orleans and Washington counties in Vermont

Coos and Grafton counties in New Hampshire

Ashtabula, Belmont, Carroll, Columbiana, Geauga, Harrison, Jefferson, Lake, Mahoning, Portage, and Trumbull counties in Ohio

Allegheny, Beaver, Crawford, Erie, Greene, Lawrence, Mercer, Washington, and Westmoreland counties in Pennsylvania

Barbour, Brooke, Greenbrier, Fayette, Hancock, Harrison, Lewis, Marion, Marshall, McDowell, Mercer, Monongalia, Nicholas, Ohio, Pocahontas, Raleigh, Randolph, Summers, Taylor, Upshur, Webster, Wetzel, and Wyoming counties in West Virginia

In October, WS will distribute more than 73,000 ONRAB baits by helicopters and vehicles in southern Tennessee (Bradley, Hamilton, Marion, and Sequatchie counties) to further evaluate the effectiveness of the vaccine with these distribution methods. Approximately 6 to 8 weeks after the bait is distributed, WS personnel will collect samples from raccoons and skunks to determine vaccination rates.

“Humans and pets cannot get rabies from contact with the bait but are asked to leave the bait undisturbed if they encounter it,” wrote the APHIS in a news release.

 “If contact with bait occurs, the contact area should be immediately washed with warm water and soap. Each bait carries a toll-free number that people can call if they have additional questions concerning a bait contact.”

According to the APHIS, the 2024 field assessment of ONRAB is a joint initiative involving the APHIS, the CDC, the vaccine producer Artemis Technologies Inc, and various state departments of agriculture, health, and natural resources.

Currently, the National Rabies Management Program is using another oral rabies vaccine, RABORAL V-RG, to control the disease in raccoons, coyotes, and foxes. To make the RABORAL V-RG vaccine appealing to animals, the vaccine sachets are either coated with fishmeal or embedded in solid blocks composed of fishmeal and polymer. According to the APHIS, in Florida, raccoon visitation rates to RABORAL V-RG bait stations have been recorded at more than 50%.

When it comes to hard days for pet parents at the veterinary clinic or euthanasia appointments, it can be a heavy weight for pet parents and veterinary professionals to carry. This can lead veterinary professionals to empathize and sympathize with clients and relate themselves to them, but is there a difference between the 2? If so, what is it and why is it important to understand?

During this interview previewing her upcoming lecture at Fetch Kansas City, Jill Clark, DVM, explained the difference between empathy and sympathy, and where they play a part in the veterinary clinic. 

Sympathy is like expressing care and concern for someone's feelings without necessarily sharing those emotions yourself. And sympathy, honestly has a little bit of a distance that we keep from the client. So sympathy might sound like, Oh, I'm sorry for your loss. Let me know if there's anything you need, right? So I feel bad for you. If there's something you need, you let me know, and I'll help you. Whereas empathy is the ability to understand and share the other person's feelings, and it often and really should have a deep emotional connection to the point, really, that we're imagining how that person would feel, or we're recalling emotions that we've had in a similar situation.

Sympathy vs. empathy

Behavior changes often manifest as metabolic diseases in dogs and cats, complicating diagnoses and treatment. Intrinsic factors, hormones and neurochemicals, and extrinsic factors such as environment and interactions with people or other animals, can reinforce or discourage behavioral changes due to metabolic diseases. For example, a dog with hyperadrenocorticism (Cushing's Disease) may start urinating indoors due to increased thirst and frequent urination. The dog reinforces this behavior by feeling relief from bladder pressure. Even after treating the underlying condition, the learned behavior may persist, necessitating further behavioral modification. Similarly, a cat with undiagnosed hyperthyroidism might vocalize at night due to hunger, ask to be fed by its caregivers. This behavior reinforces itself, and even after treatment, the cat may continue this pattern, having learned that vocalizing brings food.

Thyroid disorders predominantly show how metabolic diseases can affect behavior. Although rare, hypothyroidism in dogs can cause aggression, disorientation, and mental dullness. Hyperthyroidism, more common in cats, often results in anxiety, restlessness, increased appetite, and changes in litter box habits. Additionally, cognitive dysfunction in older pets can further complicate these behavioral changes.

Sometimes, behavioral changes in pets with metabolic diseases are the only noticeable sign, highlighting the importance of thorough diagnostic workups. These workups include physical exams, blood tests, thyroid analyses, and other relevant diagnostics.

Traditionally, veterinarians ruled out physical problems before addressing behavioral issues. However, recent research shows that behavioral and physical health are interconnected. Physical diseases can cause behavioral changes, and vice versa. Therefore, veterinarians must address both aspects simultaneously. Recognizing and treating behavioral signs immediately, along with educating caregivers, can prevent the progression of negative behavior patterns.

For effective treatment, clinicians should simultaneously address both physical and behavioral symptoms. Early intervention alleviates stress and prevents the establishment of long-term behavior problems. For instance, if a dog shows aggression when handled, caregivers should use positive reinforcement, such as treats or toys, to guide the dog rather than using physical force. Similarly, for a cat that vocalizes at night, caregivers can use an automatic feeder to anticipate and meet the cat's needs without reinforcing disruptive behavior.

With this, veterinarians improve the overall well-being of pets by recognizing and addressing both physical and behavioral symptoms early, ensuring they lead healthier, happier lives.
